I just got this expansion and I'm confused on how the game starts. The way I'm reading the rule is that the "Fistful of Cards" is always the first card in effect. Is that true? Also, am I to understand that the players not only see the card that is currently in effect, but since the stack is face up, they know what card will go into effect next round?

No, this is not true, a card Fistful of cards is on the top, but then you turn the deck upside down and now it's a last card in this expansion.

The card titled "A Fistful Of cards" is always the last (usually 13th; twelve hours till high noon...) card.
All other cards are put face-up on it.
After the first round, before the sheriff's turn (and before his dynamite and/or jail phase), the first card is put next to the AFFoC stack and comes into play.
That way, the next card is already revealed for people to see and get prepared during the current turn.

This procedure has the flaw that you could mistakenly push the stack and reveal more cards than the top one.
So me and my friends simply play with a face-down stack ("A Fistful Of Cards" still being the last one) and make it a bit more random and surprising.
